This is to report a case of the adult Fanconi syndrome, who has been admitted to the Seoul, National, University Hospital.
The patient was a 39-year-old Korean military officer, who had been suffering from severe back pain for-the last two years. The patient presented with osteomalacia, hypophosphatemia Milkman¢¥s pseudofracture, renal glycosuria, aminoaciduria without hyperaminoacidemia and renal tubular acidosis. The unusual feature in this case was the presence of the atypical plasma cells laden with bizarre shaped vacuoles in the bone marrow aspirates. Monoclonal gamma-globulins have never been detected in the serum and urine in spite of extensive efforts to demonstrate them.
To the best knowledge of the authors this is the first case of the adult Fanconi syndrome reported in Korea. The relevant literature was reviewed.
